Meet Dr. Barsam Gharagozlou, MD, FAAP
President & Medical Director

Dr. Barsam Gharagozlou, MD, FAAP, is a board-certified pediatrician dedicated to providing expert, compassionate care for newborns, children, adolescents, and young adults up to age 26. With a deep commitment to patient-centered medicine, he ensures that children receive the highest quality care at every stage of development.
A Leader in Pediatric Medicine
Dr. Gharagozlou completed his pediatric residency and internship at RWJBarnabas Health – Newark Beth Israel Medical Center in New Jersey. He began his career at Valley Children’s Healthcare in 2016, serving families in the North Valley community. During his tenure at Emanuel Medical Center in Turlock, he held key leadership positions as Chairman of the Pediatric Department and Member of Hospital Executive Committee , helping to shape pediatric healthcare initiatives.
In 2018, he relocated to the East Bay Area, expanding his practice to serve families in Antioch, Pittsburg, Bay Point, Concord, Brentwood, and beyond. Since September 2020, he has led Delta Pediatrics Medical Group as President & Medical Director, guiding a team dedicated to high-quality, personalized pediatric care.
Expanding Pediatric Care in the East Bay
Recognizing the growing need for accessible, high-quality pediatric care, Dr. Gharagozlou made a strategic decision in 2024 to acquire Diablo Valley Pediatrics, further expanding his ability to serve families across the region. Under his leadership, both Delta Pediatrics Medical Group & Diablo Valley Pediatrics continue to provide exceptional, patient-centered care to thousands of children.

Ryan Tracy, M.D., F.A.A.P.
Dr. Ryan Tracy earned his medical degree from the University of Colorado Medical School and completed his pediatric residency at the University of Florida, Gainesville. With a passion for providing comprehensive pediatric care, he has dedicated his career to the well-being of children and adolescents.
Dr. Tracy served as the President and CEO of Diablo Valley Pediatrics Medical Group before its transition. Outside of medicine, he enjoys running, biking, traveling, and outdoor activities.
Lucia Yang, M.D.
Dr. Lucia Yang is a highly experienced pediatrician with 42 years of expertise in Pediatrics and Adolescent Medicine. As a long-time member and owner of Diablo Valley Pediatrics prior to its transition, she has been a trusted provider for generations of families.
After a short period of partial retirement, Dr. Yang returned to practice to continue caring for the patients of Diablo Valley Pediatrics, after the transition, demonstrating her unwavering dedication to children’s health and well-being.
Azita Foroutani, FNP
Azita Foroutani, FNP is a Family Nurse Practitioner with a strong background in both medicine and advanced nursing practice. She completed medical school outside the United States and practiced as a physician for three years before moving to the U.S in 2006. After relocating, she successfully passed all United States Medical Licensing Examinations (USMLE) and obtained ECFMG certification. Instead of pursuing medical licensure in the U.S., she chose to transition into a career as a Family Nurse Practitioner, allowing her to integrate her extensive medical knowledge with companionate hands-on patient care.
With the combined perspective of formal medical education and nurse practitioner training, Azita brings a unique, well-rounded approach to pediatric care, emphasizing both clinical expertise and compassionate, patient-centered treatment.
Michelle Tran, CNP, RN
Michelle Tran is a Certified Nurse Practitioner and Registered Nurse specializing in pediatric care. She is passionate about ensuring the health and development of her young patients, offering both preventive and acute care with a focus on family-centered treatment.
Kaitlin Warren, CNP, RN
After graduating from UCSB, Kaitlin received her Nurse Practitioner degree at Columbia University in New York. She moved back to her hometown of Pleasant Hill. She enjoys running marathons, traveling, baking and reading. Kaitlin has two adorable cats Penny & Finn.
